SELF-MANAGEMENT GROUPS
Self-Management groups are offered, free of charge, in collaboration with Central East LHIN, to empower people to take charge of their health, while living with health conditions like diabetes, heart disease, arthritis, lung disease, chronic pain and other chronic health issues or as caregivers of individuals experiencing chronic health conditions. Groups involve weekly sessions for a duration of 6 weeks during which participants are offered a Tool Box Model and make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program or as caregivers. All resource materials provided have been reviewed by Stanford University and other health professionals in the community.

CHRONIC DISEASE SELF MANAGEMENT
Peer support workshop to enable people with chronic disease to manage their physical and mental health conditions in order to improve or maintain their health status.
CHRONIC PAIN SELF MANAGEMENT
Peer support workshop to help people with chronic pain explore healthy ways to manage and live with chronic pain condition in order to enjoy more fulfilling and satisfying lives.
DIABETES SELF MANAGEMENT
Peer support workshop to increase diabetes self-management capabilities to manage diabetes, help avoid or delay onset of diabetes complications.
POWERFUL TOOLS FOR CAREGIVERS
Peer support workshop which is designed to improve caregivers mental and physical health by improving self- care behaviours, self- efficacy and management of emotions.
