ALLIED HEALTH

Our Dietitians aim to help you prevent and manage new or existing medical conditions through nutrition counselling and lifestyle modifications. Dietitians are the health care professionals for all questions or concerns related to nutrition. Services provided by our Dietitians include 1-1 appointments and/or group settings covering an array of topics such as Pre-Diabetes, Diabetes, Cardiovascular Disease, Weight management, Nutrition through the life cycle, Gastrointestinal concerns and so much more!

Our Registered Nurses (RNs) work with your family physician and interdisciplinary healthcare team to provide one on one primary care and group education focusing on health promotion, disease prevention, and chronic disease management. The RNs are trained to provide evidence-based care and education for chronic diseases including diabetes, hypertension, hyperlipidemia and chronic kidney disease. They work in collaboration with your family physician to provide a variety of primary care services including: well baby assessments, cancer screening, and immunizations. A visit with our RNs will ensure your primary care needs are addressed and you receive the right care at the right time by the right provider.

Our Nurse Practitioners (NPs) are Registered Nurses in the extended class. NPs are trained to provide evidence based care and work with your health care team to provide the right care at the right time. They are authorized to diagnose, order and interpret diagnostic tests, as well as prescribe medication and other treatments.

Our Occupational Therapist works with you to help maintain participation in everyday tasks that are important to you. Our OT runs the sleep program, provides infant sleep education, and will conduct an intake for seniors to help navigate appropriate program recommendations for groups including, exercise, falls prevention, healthy living and memory, and senior social club. For patients registered with our physicians, cognitive screens and walker assessments are also available.

Our Clinical Pharmacist works with you and your primary care provider to help manage your medication regimen, answer medication related questions, review drug interactions and ensures you are taking your medications correctly. Our Clinical Pharmacist is involved in a variety of different programs within the FHT including but not limited to; diabetes and insulin titration, blood pressure/cholesterol management, medication tapering, smoking cessation, lung health and seniors assessments; offering evidence-based advice. Our Physiotherapist works with family physicians and the interdisciplinary healthcare team (referrals are subject to EGTA FHT service criteria) to provide 1-1 consultations, education and group exercise programs to manage pain, enhance mobility and physical fitness, encourage functional independence, and improve quality of life.

Our Registered Social Workers provide short-term counselling mainly focused on issues of anxiety and depression. Using a variety of therapy modalities, the social workers help hundreds of patients each year to cope with challenging life circumstances and improve their mental health.

ADMINISTRATION

The executive director is responsible for the strategic and operational planning and management of the FHT while supporting the Board Chair and the Lead Physician.

East GTA FHT was created by Dr. C. Jyu with support from Sudin Ray to serve the patients in the Scarborough region; they were one of the first offering team based primary care to practicing physicians in the region. This legacy of equitable care was later adopted by the province in a legislation. They also pioneered the complex patient care model by aligning home care with primary care to help reducing avoidable visits to acute care facilities by seniors in our care.

The role of the Board Chair, Dr. A. Vellathottam, is to ensure good governance of the not-for-profit corporation and more importantly, compliance with the funding agreement signed with the Ministry of Health. East GTA FHT was also one of the first FHTs adopting a competency-based Board to ensure that the Board has necessary competencies to effectively carry out its duties and responsibilities while maintaining collegiality and due diligence.

The lead physician, Dr. W. Hsieh, supports the development of population-based programs, and provides clinical guidance to the allied health professionals.

Our Manager aims to ensure providers within our team have what they need to focus on patientcare. This includes but is not limited to payroll, benefits administration, ordering and maintaining supply inventory, ensuring technology is running smoothly, coordinating surveys and statistics and reviewing referrals to FHT programs.

Our receptionist is an integral part of our team. She helps coordinate appointments for all of our allied health providers improving access to patient care, reviews referrals, addresses and answers any questions/concerns and provides an overall welcoming environment. 

Our Quality Improvement Decision Support Specialist assists the family health team in data reporting and also help in clinical decision making through quality improvement initiatives, patient centered programs and services and other quality improvement activities.
