The Heart Health Program offers individual counselling and group presentations to patients with elevated blood pressure and/or cholesterol levels.

Individual visits are also offered with either a Registered nurse for assessment and education, Registered Dietitian for an individualized diet plan, and Pharmacist for medication review.

Bloodwork is required upon referral.
