HOW TO ENROLL AS A PATIENT
To enroll as a patient with one of our family physicians, please call the number associated with the physician (click here) to see if they are accepting new patients and to book an appointment. You MUST have a valid OHIP card to register.
When coming to your first appointment please bring the following:
- List of your most current medications (you can obtain this from your pharmacy)
- Valid OHIP card
- Immunization record
- A translator if needed
You will need to fill out registration and enrollment forms so please arrive 15-30 minutes before your appointment time.
To enroll in a group workshop or for individual services provided by the interdisciplinary team, you do not need to be registered with a physician. View our program guide for a list of group and individual sessions the team offers and call 647-693-7401 for more information or to register.
