SENIORS WELLNESS PROGRAM

A comprehensive geriatric assessment is designed for individuals 65 years or older to help identify any medical, psychosocial, and functional limitations of an older person in order to develop a care plan to maximize his/her overall health with aging.

The Nurse Practitioner will assess and evaluate an older person’s functional ability, physical health, memory/cognition, mental health, nutrition, immunization, sleep, bone health, and other socioenvironmental circumstances.  Further assessment, evaluation, and management can be done if any potential problem has been identified. The Nurse Practitioner may refer you to other healthcare providers (e.g. social worker, dietitian, pharmacist, physiotherapist, and/or occupational therapist) for further assessment or support/treatment as needed. The Nurse Practitioner will also work with your Primary Care Provider and keep them informed to provide the best care.

Regular follow-ups are also provided by the Nurse Practitioner.

Please bring your medications, including the ones you take over-the-counter to your appointment.

Our seniors program also comprises of groups and education services. An intake assessment is conducted by our Occupational Therapist who reviews your needs and is able to offer guidance on appropriate groups and programs. Please see below for descriptions of our current program offerings.

GROUPS

HEALTHY LIVING & MEMORY WORKSHOP

This workshop focuses on healthy living for memory fitness. It is suitable for individual who has self-complaints of memory concerns or who is interested to learn about brain health. Topics include:

  • Memory and aging
  • Risk factors affecting memory
  • How to preserve memory by healthy living
  • Memory strategies

OLDER ADULT EXERCISE GROUP
This exercise program focuses on cardiovascular and muscle conditioning exercise. There are 3 levels which involve chair exercises and/or standing exercises. Home exercise is provided. Group consists of 6 sessions of 60 minute exercise.

FALLS PREVENTION
This multidisciplinary workshop includes review of falls from 4 different perspectives: dietary, medications, aging and physical health, and environmental/behavioural factors. Each session also includes balance and strengthening exercise led by a physiotherapist. Group consists of 4-weekly sessions for 90 mins.

SENIORS SOCIAL CLUB

This 2-hour group is a great way for you to interact with other seniors within the community. It comprises of health education and light exercises led by our physiotherapist. It is offered online and in the clinic. New group starts in January, May, and September, once monthly for 4 sessions.
